The Prenatal Care Coordination team - composed of social workers, registered nurses and community health workers - provides comprehensive services to women with high-risk pregnancies. Once a woman's needs are assessed, team members provide individualized education, support and coordination of services to maximize healthy pregnancy outcomes and decrease infant mortality and morbidity.
Prenatal Care Coordination teams are located at Wishard Memorial Hospital, Westside and Forest Manor Community Health Centers and certain Marion County Health Department locations. Services are provided by referral through all Wishard OB clinics.
